In consideration for the provision of the services listed on Schedule A (“Services”) by the Urban Dog, Inc., to the pet(s) indicated on Schedule A, for the Client, and intending to be legally bound, the undersigned client (“Client”) agrees as follows:

    Pet walking / In-home service

  1. The Urban Dog, Inc. will provide the Services to the Client in a reliable, trustworthy, and timely manner. Schedule A can be amended by The Urban Dog, Inc., from time to time.

  2. The Urban Dog, Inc. will use reasonable efforts to provide such services within two (2) hours of the time designated for the provision of such Services during The Urban Dog, Inc.’s business hours.

  3. The Urban Dog, Inc. will walk pets only on leashes.

  4. The Urban Dog, Inc. will not walk pets that are aggressive, unruly, untrained, or that choke themselves on their leashes.

  5. The Urban Dog, Inc. is not responsible for any keys that the Client has asked The Urban Dog, Inc., to mail, or inability to gain entrance to the location of the pets, or any other situation not in the control of The Urban Dog, Inc.


    6. Daycare and Overnight service

  6. Your pet will be interacting with other dogs at The Urban Dog, Inc and you understand the risks involved to your dog(s) by such interactions. I assume all these risks in respect to my dog(s)


    7. All Services

  7. The Client understands that The Urban Dog, Inc., does not diagnose or treat medical conditions or ailments of any sort, human or animal and does not provide veterinary services. If the Client is unavailable, or if there is an emergency, The Urban Dog, Inc., will refer any observed medical concerns to a veterinarian at the Client’s cost. If the Client’s veterinarian is available, The Urban Dog, Inc. will refer the medical concerns to the Client’s veterinarian.

  8. The Client will pay The Urban Dog, Inc. for the Services in advance. The Client will pay any additional charges, including, but not limited to, emergency food, supplies, and veterinarian’s fees, within five (5) days of the charges being incurred. A late fee of twenty dollars ($20) per week will be charged with respect to all late payments.

  9. The Client will provide two (2) days advance notice of any cancelled Services. The Client will pay a thirty dollar ($30) cancellation fee if two (2) days advance notice is not provided.

  10. The Client will provide The Urban Dog, Inc. with contact information for a responsible party (such as a neighbor) capable of caring for the pets in the event of inclement weather or an emergency (such as the illness or incapacity of the Client or the employee of The Urban Dog, Inc.), or any other situation not under the control of The Urban Dog, Inc., to provide for the safety of the pets.

  11. Each pet will have a veterinarian and will be up to date on all required or recommended vaccinations.

  12. The Client will provide The Urban Dog, Inc. with complete and accurate contact, pet, veterinarian and other information in a timely manner, including the information requested on Schedule B. CLIENT UNDERSTANDS AND ACKNOWLEDGES THAT IT IS THE CLIENT’S RESPONSIBILITY TO UPDATE THE INFORMATION IN A TIMELY FASHION AS THE INFORMATION CHANGES. ACCURATE CONTACT AND VETERINARIAN INFORMATION IS ESSENTIAL FOR THE PETS’ COMFORT AND SAFETY.

  13. The Urban Dog, Inc. reserves the right to deny or terminate Services for any reason or no reason. This Agreement may be terminated at any time by either client or The Urban Dog, Inc. The provisions for payment and cancellation and late fees will survive termination by either party.

  14. THE CLIENT EXPRESSLY WAIVES ANY AND ALL CLAIMS AGAINST THE URBAN DOG, INC. AND ITS EMPLOYEES, UNLESS ARISING FROM NEGLIGENCE ON THE PART OF THE URBAN DOG, INC.

  15. THE CLIENT AGREES THAT THE URBAN DOG, INC. WILL NOT BE LIABLE FOR THE LOSS, INJURY, DEATH, OR ACTIONS OF ANY PET THAT THE CLIENT HAS LET OUTSIDE OR INSTRUCTED THE URBAN DOG, INC., TO ALLOW OUTSIDE IN THE ABSENCE OF AN EMPLOYEE OF THE URBAN DOG, INC., (INCLUDING OUTDOOR PETS AND PETS UTILIZING PET DOORS).

  16. THE CLIENT WAIVES THE RIGHT TO A TRIAL BY JURY OF ANY CLAIM ARISING UNDER THIS AGREEMENT OR IN CONNECTION WITH THE PROVISION OF THE SERVICES.

  17. The provisions of 14, 15 and 16 will survive termination of this Agreement.


By signing below, the Client acknowledges that he or she fully understands and agrees to all of the terms of this Agreement.
